Thanks to Jonathan for the research on this. The answer [again, when the base standard has a Purpose clause] is:

If the amendment has no Purpose clause. then the PAR should answer:
 5.4 Will this document contain a purpose clause? YES

I think the answer is No or Yes, depending on whether "this document" is the amendment (which has no purpose clause) or the amended standard (which retains the unamended one).

The Purpose clause is the one on the standard. An Amendment Par that is changing the Purpose statement could do so here. 
The scope statement was split to an "a" and "b" so that the amendment PAR could have a scope of the amendment and be able to update the scope of the draft itself if needed.

A paragraph describing the purpose of the standard is not mandatory in the draft. However, if included, the purpose of the standard and its intended application shall be included in a separate subclause (1.2). The purpose shall explain why the standards project is needed.
If the document will not include a purpose statement choose "No" and leave the purpose field blank. 
The Purpose stated on the PAR shall be written in present tense, in complete sentences, and with proper grammar as it is intended to appear in the published standard. The title and (if appropriate) date of any document referenced in the Purpose shall be listed in the Additional Explanatory Notes field at the end of this PAR form.
Take the case in which the PAR is for an amendment of a standard that already contains a Purpose statement and the intention is to leave the Purpose unamended. Then I can understand the highlighted sentence two ways:

(1) If “this document” means “the amended standard”, then the answer should be “Yes”.
(2) If “this document” means “the amendment”, then the answer should be “No”.

Which is the correct understanding? Also, is this understanding apparent from the instructions?
