[STDS-802-16] release of P802.16-REVd/D5

P802.16d/D5 is now available:

and a version showing the markups as compared to D5:

I have submitted the last one for a second 15-day Sponsor Ballot
recirculation, which opened today (Friday).

The editor, Itzik Kitroser, worked extremely hard on this. Please
