[802.19] Update of comment spreadsheet on IEEE 802.19.3a D01

Dear TG3a members,

I updated the comment spreadsheet on IEEE 802.19.3a D01 to r4 reflecting the discussion in the Thursday TG3a session.

The following updates are made. (They are highlighted by blue color.)

* Add the agreed resolutions (CID 111 By Nagai-san, and CIDs 54, 58, 74, 123, 147, 148, 149, 150 by Kitazawa-san).

* The resolution for CID 38 is updated.

* The document number 19-29/0011r2 is updated to 19-29/0011r3.

* The agreed dates are added for the agreed resolutions.
