When posting to Mentor the procedure is documented here:

should have the first author's name if that person is posting and
affiliation in the "Author (Affiliation):" field with the affiliation in
parenthesis.
Note that you should also have that as the lower right hand footer for the documents.
and in the left hand footer it should say the document type...in your case "Submission".
If you post an updated revision I request you consider updating the footer correctly, and follow the template for future documents.
