[STDS-802-16] Proposed Tutorial on 802.16 Operator ID for IEEE Registration Authority

At the 802.16 WG's Session #43 in May, it tasked an ad hoc committee
to draft a proposed tutorial on 802.16 Operator ID for consideration
by the IEEE Registration Authority Committee.

It provides tutorials for both of the options as defined in our
letter (IEEE 802.16-06/016r3), with two versions of the second option.
