Member and Observer Registration Form:
IEEE 802.16 Session #35 (Sanya, Hainan, China)

Use this form to register for IEEE 802.16 Session #35 (24-27 January 2005) if you are a Working Group Member or Observer.

(1) Provide select your name.
If you aren't listed, see the Nonmember Registration Form. If you need to update your 802.16 contact information, use the IEEE 802.16 Member/Observer form.

(2) Select the action you wish to take:

Register for the Session. Your registration fee will be payable on-site by credit card. The fee will be US$160, payable to the hotel by credit card, for those preregistering with this form by 5 January; afterwards, the fee climbs to $200. The fee is now $200, payable to the hotel by credit card. Please cancel your registration immediately if your plans change!
Cancel registration for the Session.

(3) Please answer the following:

Do you plan to stay at the International Asia Pacific Convention Center HNA Resort Sanya?No Yes. {If so, you should reserve a room immediately}.

Will you attend the social on Wednesday night? No Yes

How many guests (excluding yourself) will you bring to the social?

Will you attend an introduction to the IEEE 802.16 Working Group on Monday morning? No Yes
