stds-802-16-tg4: work on Multiple Access Methods for TG4

Hi all,

Let's start gathering inputs on 'Multiple Access Methods for TG4'. This
addresses issues such as polling and ranging.

As far as I know, we have so far three approaches to consider:

1. CDMA based ranging (RunCOM)
2. Subcarrier based Polling (BreezeCOM)
3. Plain old Slotted ALOHA

I suggest we start by defining the goals of the polling and ranging
mechanisms. Then we can we move to the actual evaluation.

Please submit any ideas/ inputs you have.
